Medical Requirements for Intercollegiate Athletics Participation

Welcome Student-Athletes!

The Saint Joseph’s College Athletic Training Staff looks forward to assisting you in your athletic endeavors and we will strive to provide an optimal environment in which you can succeed.

**The following information is intended for returning varsity Student-Athletes and for those Student-Athletes (incoming 1st year or new athlete) who are currently on a roster or expected by the coaches of the respective teams.

Contact the Saint Joseph’s College Athletics  Department for information about  intercollegiate athletics eligibility and participation.

In order to participate in intercollegiate athletics at Saint Joseph’s College, the following medical requirements must be satisfied:

  • Pre-Participation Physical Evaluation (PPE)
  • Electronic Pre-Participation Evaluation (e-PPE)
  • ImPact Baseline Testing
  • Injury and Illness Reporting and Acknowledgement Form
  • Sickle Cell Trait Testing 

DEADLINE FOR COMPLETION - FALL ATHLETES: 8/7/2015     WINTER & SPRING ATHLETES: 8/21/2015


Pre-Participation Physical Evaluation (PPE)

Beginning in the fall sports season of 2015, a new Pre-Participation Physical Evaluation (PPE) system will be in place.

All fall sport PPEs will be conducted by the SJC Sports Medicine team under the direction of Maine Medical Center Sports Medicine Program and will be held in the Athletic Training Room at Alfond Center on August 19th, 2015 (team times TBA). Please dress accordingly (workout attire) for your appointment.

PPEs for Winter/Spring athletes will be held on Wednesday, September 2nd (team times TBA).

ALL prospective 1st time or transfer student-athletes:  MUST complete an initial, comprehensive Pre-Participation Physical Evaluation (PPE) with a Saint Joseph’s College team physician and complete the following e-PPE health history and questionnaire (**see e-PPE next section).

ALL returning student-athletes: MUST complete a PPE update with the SJC Sports Medicine team and complete the following e-PPE health history and questionnaire (**see-PPE next section). Note: Returning student-athletes whose PPE update indicates a need to follow up on a resolved or unresolved medical condition must also be seen by the SJC team physician for clearance to participate. 

 


Electronic Pre-Participation Evaluation (e-PPE)

All student-athletes MUST complete the electronic Pre-Participation Evaluation (e-PPE) medical history questionnaire prior to being seen by the SJC Sports Medicine team. The e-PPE is a comprehensive, branched-chain questionnaire which will allow you to present detailed responses regarding your medical history. This web-based questionnaire is on a highly secure website (PrivIT), which is compliant with International privacy laws including HIPPA, PIPEDA and FERPA. Each response to the questions in your personal health profile is encrypted and viewed only by sports medicine personnel (including: athletic trainers and SJC team physicians).

  • For all fall student-athletes: The e-PPE form must be completed by August 7, 2015
  • For all winter & spring student-athletes: The e-PPE form must be completed by August 21, 2015
  • If you are under age 18 when completing the e-PPE you must have your parent/legal guardian assist you with this process and sign your documents  
  • Coaches will be able to monitor the progress toward completion by their student-athletes from their respective team. Coaches will only have access to insurance information and emergency contact information as well as awareness of medical conditions that may affect participation. 
  • Only SJC Sports Medicine team members will have access to any e-PPE Health History Summary 
  • Please be very thorough with your responses as you complete the questionnaire
  • BE SURE TO PRINT YOUR COMPLETED DOCUMENTS WHEN YOU ARE FINISHED!! YOU NEED TO BRING THEM AND YOUR MEDICAL INSURANCE CARD WITH YOU TO THE PPE APPOINTMENT!!
**Important: The PrivIT website link which follows will enable access to the Welcome page of the PrivIT Profile and guide the student-athlete through the registration and questionnaire process. This process should take approximately 45 minutes to complete for first time registrants.

The Welcome page will also describe the information you should have on hand to complete the questionnaire. Please be very thorough and make use of the suggested “Get Help” how-to guides and tutorials which are readily accessible. These guides are very detailed, user friendly and describe thoroughly the registration process and the e-PPE questionnaire. They are a valuable resource!

ImPact Baseline Testing

Each prospective First-time or transfer student-athlete is required to complete ImPact online, baseline concussion testing before participating in athletics at Saint Joseph’s College. The ImPact testing will be administered by the SJC Medical team to those notified athletes upon completion of their PPEs.

 


Injury and Illness Reporting and Acknowledgement Form

Each fall student-athlete is required to submit a signed Injury and Illness Reporting and Acknowledgement Form which will be provided during an informational meeting to be scheduled as part of the PPE process on August 19th. There will also be 10 minute concussion education video presentation during this meeting. Student-athletes are required to attend this meeting and view the video before participating in their sport.

NCAA CONCUSSIONS FACT SHEET

 

Sickle Cell Trait Testing

The NCAA requires all first year and new student-athletes participating in athletics show evidence of Sickle Cell Trait Testing and receive education regarding this condition. Returning student-athletes who have completed the testing will not be asked to repeat this process.
