Required Forms for Student-Athletes:All forms are due prior to each seasonal Sports Physical date here at Saint Joseph's College. Those athletes who do not submit their forms prior to the scheduled date will be required to reschedule with the Sports Medicine Team. Incoming/Transfer Student Athletes: Returning Student Athletes: 2012-13 NCAA Banned Substance List The Saint Joseph's College of Maine Department of Athletics requires that all student athletes take part in a Pre-Participation Sports physical held by our Sports Medicine Team and staff prior to participating in athletics. Physicals will be held prior to the start of each athletic season and will cost student athletes $25.00. Those athletes participating in multiple sports are only required to get one physical per school year. |

Neil K. Carroll, ATC, LAT, PES, FNS,
FMS-L1, CGFT A native of Gray, Maine and 1997 graduate of Gray-New Gloucester High School, Carroll received a B.S. in Athletic Training from University of New England in 2002. He worked as the Head Athletic Trainer at Greely High School during the 2004-05 academic year and served in the same role at Scarborough High School for six years. Carroll holds two different certifications from the National Academy of Sports Medicine, which include Fitness Nutrition Specialist (FNS) along with his Performance Enhancement Specialist (PES). Carroll also is certified in Functional Movement Systems (FMS Level 1) and holds specializations in both Golf Fitness (CGFT) as well as a Cancer Exercise Specialist through the Cancer Training Institute. At this time Neil is currently enrolled in the Masters in Education program here at Saint Joseph’s College of Maine. Neil resides in Gray with his wife Amanda and their two daughters Emily and Molly. |

Tiffany Miller-Gagnon,
MS, ATC, LAT, PES The 2011-12 academic year will be Tiffany Miller-Gagnon's seventh as an Assistant Athletic Trainer and second as the SWA at Saint Joseph's. Prior to St. Joe's she completed a year-long Graduate Assistantship as a Head Athletic Trainer at California Area High School in California, Pennsylvania. Miller-Gagnon graduated from University of Maine-Presque Isle with a B.S. in Athletic Training in 2004 and attained her Master's Degree from California University of Pennsylvania in 2005. She is a member of both the National Athletic Trainer's Association and National Academy of Sports Medicine and also is a certified Performance Enhancement Specialist. Along with her duties as the Assistant Athletic Trainer, Miller-Gagnon also serves as the Senior Women's Advisor at St. Joe's and works with the SJC Student-Athlete Athetic Advisory Committee. Miller-Gagnon is a native of Caribou, Maine and resides in South Portland, ME with her husband Jeff. |
