The Saint Joseph's College of Maine Department of Athletics requires that all student athletes take part in a Pre-Participation Sports physical held by our Sports Medicine Team and staff prior to participating in athletics. Those athletes participating in multiple sports are only required to get one physical per school year.

Welcome to Monks Athletics 2023-2024. Please take the time to fill out this form to the best of your ability. Be honest and accurate with your responses. By answering with integrity, it will allow our medical team to protect and provide you with the best opportunity to fully participate. In addition to filling out the PPE GOOGLE FORM, please upload signed copies - using the upload buttons on the PPE Google Form - of the following:
- Sickle cell document (1st years/Transfers only) - FORM
- Authorization to Exchange Information document - FORM
- Injury & Illness Reporting document - FORM
- A copy of the front and back of your current insurance card
** If you are a minor, please have a parent/guardian sign the form as well **

ATHLETIC TRAINING ROOM
The Athletic Training facility is located in the Harold Alfond Center adjacent to the gymnasium.
Athletic Training services will be available before and after all athletic contests. Pre-game preparation will be provided to visiting athletes; however we do ask that you provide your own supplies. A Certified Athletic Trainer will be present at all home contests. Those teams traveling with a Certified Athletic Trainer will have full usage of the athletic training facility but are asked to make SJC Athletic Training staff aware of any special arrangements one may need.

HOURS OF OPERATION:
During the academic year, the athletic training room will be open 12:00 PM - 3:00 PM, Monday - Friday, for rehab, treatment, and evaluations.
Athletes who have taping needs should arrive at the training room no earlier than an hour prior to the start of their practice session.
For events, athletes will have access to the athletic training room two hours prior to the start of their contest.
On weekends during the academic year, the athletic training room will be open an hour before the first practice or two hours before the first game of the day and will close after the final practice or game has ended.
