Welcome to Monks Athletics 2023-2024. Please take the time to fill out this form to the best of your ability. Be honest and accurate with your responses. By answering with integrity, it will allow our medical team to protect and provide you with the best opportunity to fully participate. In addition to filling out the PPE GOOGLE FORM, please upload signed copies - using the upload buttons on the PPE Google Form - of the following:

• Sickle cell document (1st years/Transfers only) - FORM
  
  
• Authorization to Exchange Information document - FORM
  
  
• Injury & Illness Reporting document - FORM
  
  
• A copy of the front and back of your current insurance card

** If you are a minor, please have a parent/guardian sign the form as well **
